Photo / Video Consent Waiver
I grant permission to Please Pass the Manners to use photographs and/or video taken of my child during any Please Pass the Manners event for use in publications, website, and other forms of media without notifying me. I hereby waive any right to inspect or approved the finished photographs or printed or electronic matter that may be used in conjunction with them now or in the future, whether that use is known to me or unknown, and I waive any right to royalties or other compensation arising from or related to the use of the photograph(s). I am the guardian of my child and hereby have the right and power to submit this release.
